Participant’s environment

Did the examinee’s environment appear suitable for the assessment (e.g., quiet, distraction-free)?

## Distraction free for most part. I think her window was opened because heard noise during few of tasks.
##  - Cat in room, noise from gardeners came in near end of session.
##  - Had fan on where I can hear it spinning. Had to ask to turn it off during word inhibition.
##  - Chimes would ring every hour. Dogs barking, someone washing dishes.
##  - Would hear notifications on her computer ~ 5 times
##  - Would hear notifications from his phone ~ 5 times

Stadardized Assessment

Did you adhere to all standardised testing procedures and rules?

## Error in instructions for item 6 of Oral Comp/Recep.
##  - Should've asked her to close windows in beginning. Only asked her to put phone on silent.
##  - List-Learning Forced Choice - I read aloud the first set of word pairs, but the remaining word pairs were simply shown to pt without reading aloud.    Shape Addition - I added 'There are four potential rules'.
##  - Auditory Story Memory - accidentally said 'head-ache' instead of 'stomach-ache', but quickly corrected myself. Might have affected her delayed response.
##  - Didn't record times, could've written words neater.
##  - Mental navigation: asked if he could use numbers twice; said no when I should've said 'rules do not allow me to answer that.' Also, don't think I was supposed to replay items on Word Inhibition task. Did it because he couldn't hear them well.
##  - Spent 1 hr trying to get her web camera working. Asked her to click on camera on/off option, to check settings on Teams (device settings), check her settings on computer (privacy settings), but no luck. Offered to reschedule but she still wanted to participate, so went ahead with it. Maybe should've just cancelled and rescheduled. Skipped oral comprehension and written comprehension, first two prompts from L/R orientation, and last task (write a sentence task). For L/R orientation, still carried out task, just emphasized in beginning that she keep her head straight. She said she understands and very compliant given web camera wasn't on.
##  - List learning delayed recall: had to spell out one of the words to her (CHIN) because she thought I was saying cheese.
##  - Subitising: she couldn't get into full screen mode. Asked if I was covering screen, said no, and continued task.

Unusual Events

Did any unusual event(s) occur during testing that may have interfered with testing outcomes?

## Sometimes, pt's partner could be seen walking in the background, though pt was not distracted/looking around as they did so. Sometimes there were background noises (e.g., kettle boiling), but did not appear to interfere.    Subitizing - black screen made it difficult for pt to see as their reflection due to the sunlight was slightly obscuring the test items.
##  - Window was opened in beginning where car drove past and interrupted one of repetition items. She closed window and no distractions happened after that.
##  - Two instances of kookaburra sounds. Also heard running water but for short period.
##  - There was a moment when pt's wife walked in, and pt paused partway through WM Updating task item to let wife know to be quiet as testing occurring    Visual Learning: MS Teams task bar (appears in the centre of the screen) appeared during Trial 1 recall. The participant stopped to remove this from view, which distracted them and they had difficulty recalling items and appeared quite flustered by this.
##  - husband would walk in to watch but did not distract her.
##  - Explained that he has sore neck and is in pain, had to get up to get tablet/drink. Also, his gardeners came in (unexpected) and there was just a lot of machine noises in background during shape addition task.
##  - Called up to remind him that we had testing session today. Said he was looking at the wrong week on his diary. Offered to re-schedule but he was happy to complete it at 2:30pm. Original time scheduled was 1:30pm.
##  - Web camera not working/could not configure it to be on - no responses on some tasks. Said this camera issue doesn't happen when she is on Skype but happens when she is on Zoom and now MS Teams.
##  - Her web camera was blurry. Still could see her, but meant that she had to read aloud her sentence for written expression task.
##  - Pen ran out of ink during picture description task so might have missed a sentence from her.
##  - Asked to put phone on silent in beginning and asked again when I kept hearing his notifications.

Behavioural Observations

Behavioural observations of the participant (e.g., mood, concentration, motivation, testing behaviour, etc.)

## Generally good mood and engaged. There were times the participant would appear tired, or sigh, but they persisted and continued with effort.
##  - Motivated and concentrated and no distractions in room. Enjoyed grid task, laughed throughout it. Was in good (mood?), seemed excited to be involved in this.
##  - Participant was alert for most part. Only in dot counting (WM/updating) that I had to repeat instructions to her. Motivation was high (really invested in shape addition task). Laughed throughout the grid task (Visual Learning). Seemed happy to be involved in this, asked we donate her reimbursement money.
##  - Participant was generally very engaged. They expressed difficulties during the L-R Orientation, Mental Navigation, and Shape Addition. They were sometimes distracted by certain events (e.g., wife coming home; not knowing how to remove MS Teams task bar blocking screen).
##  - Good mood, concentrated and not seem distracted. Did admit that near the end she felt a bit fatigued such as Shape Addition task. Laughed throughout grid task.
##  - Concentrated & motivated. Might've been in bad mood in beginning (said that she volunteers for ambulance/hospital and had to take late night shift day before). Sped through first few tasks but as testing session progressed, seemed to enjoy them (less?) and find the experience (less?) amusing.
##  - Said he had neck pain and pain in general. Noticed he rubbed eyes a lot, needed to clean glasses a lot. Motivated yes, but easily distracted - would pet his cat and not pay attention to screen, took two cigarette breaks, requested a lot (3) of breaks but put effort into shape addition task.
##  - Motivated & contentrated. Seems to be an active person (likes to bicycle), would stand up during some tasks, sat down in others. Cut up oranges to eat during story reading task. Mentioned he didn't have lunch before this. Took couple tea breaks ~ 5 mins.
##  - From audio, she was overwhelmed & anxious from not having web camera on. Also had to call her on phone to help navigate through Teams software. But once we started test, she sounded very motivated & concentrated & willing to do best she can despite her camera not being on.
##  - Motivated & excited. Might have been distracted a couple times (e.g., husband arrived home), but would apologise and read instructions herself (still made sure she was paying attention). Responses were quick, finished ~ 30 minutes early. Found whole session to be enjoyable.
##  - Concentrated & motivated for most part. Would easily give up when task got more difficult (e.g., working memory/updating task). Said she takes medication for her arthritis that messes with her cognition. Would ask when task/session was going to end a lot of times but still gave 100% in responses/tried. Sensed frustration on some memory tasks (e.g., working memory, list learning, and mental navigation).
##  - Concentrated & motivated. Seemed to be in a good mood. Enjoyed grid task and list learning tasks. Did not enjoy working memory/updating task as she gave up easily and didn't really want to take her best guess for a few items.
##  - Concentrated & motivated. Laid-back person, understood instructions very well and was able to finish early despite having started 30 minutes late from technical issues. First cognitive testing experience and seemed to enjoy tasks, particularly shape addition and grid task.
