Overview: research questions

1. What is the actual and perceived level of knowledge of paramedics concerning physiological birth and associated pregnancy and/or birth complications?

Perceived knowledge questions:

  • What percentage of participants fell into the weak, adequate, and excellent for each domain?

  • Was there anything interesting about the gender, level of education, years of service or job title, when measured against the perceived knowledge rating.

Actual knowledge questions:

  • What percentage (and n=) of people scored in the weak, adequate, or excellent category for each domain?

  • For each participant, was their perceived knowledge reflective of their actual knowledge?

  • Were there any correlations (?) between gender, age, level of education, years of service or job title, when measured against the actual knowledge rating (for discussion section) but in terms of analysis would this be a correlation/multiple regression?

2. What is the perspective of paramedics regarding their initial and continuing education needs for the management of obstetric emergency responses?

Attitude – Likert scale

  • What is the spread in responses for each question – as a percentage (and n=)

  • Correlation between perceived knowledge / actual knowledge with

    • Q37_1

    • Q37_4/6/7

    • Q37_8

    • Q37_12/17/18/19

  • When a person stated strongly agree or agree in Q37_12 and Q37_13 were they more likely to score better in the actual knowledge domains.

3. How confident are paramedics in providing care to patients experiencing out-of-hospital obstetric complaints?

Preparedness – Likert scale

  • What is the spread in responses for each question – as a percentage (and n=)

  • Correlation to overall confidence level Q38 with

    • Gender

    • Years of service

    • Education

  • If a person a stated strongly agree or agree in Q38_3, did they also score well in the actual knowledge in all domains – correlation

  • As above for Q37_5 neonatal

  • If a person a stated strongly agree or agree in Q38_4, did they also score well in the actual knowledge in all domains
  • If a person a stated strongly agree or agree in Q38_2 how many also stated strongly disagree or disagree to Q38_9 (correlation between feel adequately prepared to deal with obstetric cases and need for blue light to hospital)
  • If a person a stated strongly disagree or disagree in Q38_2 how many also stated strongly agree or agree to Q38_9 (as above)
  • If a person a stated strongly disagree or disagree in Q38_8 how many also stated strongly disagree or disagree to Q37_4
  • If a person a stated strongly disagree or disagree in Q38_8 how many also stated strongly disagree or disagree to Q37_1
  • Once we have the descriptive data a separate and explicit section on ‘knowledge gaps’ may be useful to summarise i.e., a summary table of strengths and weaknesses if possible? Although this may be clear anyway as the data becomes available - will need to wait and see.

Dataset overview

The dataset contains 435 records for 91 variables. The percentage of completed (100% responses) questionnaires is \(\approx\) 64.83%, with \(\approx\) 71.72% completing at least 81% of it. More on missing data in the next section.

Completion time ranged from (rounded values) 0 to 7102 minutes, with a median of 21 minutes (first quartile 11, third quartile 139, IQR \(\approx\) 128). There seems to be little or no linear correlation between completion percentage and duration (Pearson’s \(r\approx\) -0.05).

The first 10 variables of the dataset contain metadata on completion, progress, respondent id, etc.

Data collection: The observations were collected online, using a questionnaire developed for the occasion.

Missingness analysis and final dataset

The dataset contains 10963 missing observations, which is \(\approx\) 27.69% of the total (435 rows \(\times\) 91 columns \(=\) 39585). Approximately 89.01% of the variables contain at least one missing value, as do 100% of the individual records.

Observations might not be missing in a sparse way. The next figure contains a visualisation of “where” in the dataset information is present (grey) or missing (black).

There appear to be patterns of missingness related to blocks of questions (vertical black dominance) or specific individuals (horizontal black dominance). A quick inspection reveals that the questions with the highest missingness are open-text ones (typically, “Other, please specify”). As such, these are more suited to a qualitative investigation, and will therefore be excluded from the subsequent analysis; specifically, questions 'Q2_11_TEXT','Q3_12_TEXT','Q8_9_TEXT','Q6_7_TEXT','Q11_1_TEXT' and 'Q11_2_TEXT' will be removed from the dataset. This results in the following missingness picture:

Focussing on individual-level missingness patterns, we need to compare those respondents who did not complete the relevant bits with the rest of the sample (ideally, the target population). Specifically, related to perceived (Q12_1-Q12_6) and assessed (Q13-Q36) knowledge, as well as perceived education (Q37_1-Q37_20) and confidence (Q38_1-Q38_15). We will have to exclude from the analysis those respondents presenting a too large missingness (not enough information to go on) and possibly impute some of the missing values for those with moderate/low missingness, but we certainly need to check whether and how these individuals are different from the complete/high respondents.

For reference, there are 221 records (approximately 51% of the sample) with no missing values. The following plots explore some characteristic of non-complete respondents with reference to the aforementioned “complete” group.

Note: there are many sub-analyses that can be run here re missingness. I’ll carry them out before getting to the results interpretation stage.

The final dataset to be included in the study will be comprised of those individuals that provided enough information for us to tackle the research questions. Checks to identify potential bias sources will need to be carried before the writing-up of results (see previous comments in bold). The criterion used will be to include only those who have:

  • no more than one missing value over perceived knowledge (Q12_1-Q12_6) and
  • no more than three missing value over perceived knowledge (Q13-Q36) and
  • no more than three missing value over perceived knowledge (Q37_1-Q37_20) and
  • no more than three missing value over perceived knowledge (Q38_1-Q38_15).

Furthermore, only records with reported education levels Diploma, Undergraduate and Post-graduate will be considered (leaving out those who responded Other). This resulted in the following missingness scenario:

A “quick’n’dirty” single imputation approach (replacing missing values with the mode of the corresponding variables) using the mode for each variable is employed to obtain a complete information dataset [we might want to revisit this later on]. The final dataset is comprised of 264 complete (some partially imputed) records.

Perceived vs assessed knowledge

In the following, r_s denotes Spearman’s correlation. The heatmaps report the joint (relative) frequency: the darker, the more frequent the pairing. For purely esthetical reasons, each frequency scale (the bar to the right of each plot), is capped at 0.75.
