This report tracks SPINAPS (SPN40), a study of synaptic density measured with [18F]SynVesT-1 PET in emerging and young adults with autism (with impaired or intact neurocognition) and typically developing controls. It covers recruitment, progress towards the PET targets, the data shared from the studies participants came from (SPIN-ASD, SPIN-R) and their clinical and cognitive scores.
For questions about the methods or code, contact Hassan Abdulrasul (hassan.abdulrasul@camh.ca).
How this is counted. Each participant is counted
once, at the furthest point they have reached: enrolled (has a SPINAPS
record), eligible (Eligibility Checklist says eligible), PET done (PET
Scan Information form completed), then PET usable on QC. A PET whose QC
form is still empty is shown as QC pending rather than
unusable, because no QC decision has been made yet. Targets count usable
PETs only, because the protocol replaces participants whose PET cannot
be used. The pilot target (Aim 1) is 10 per group; the full sample (Aim
2) is 28 ASD-Impaired, 27 ASD-Intact and 29 TDC
(config/targets.csv).

| Notes. Counts are participants. PET usable = reconstructed image judged usable on QC; QC pending = PET done but the QC form is not filled in yet. Targets count usable PETs only. Not assigned = no group recorded on the Eligibility Checklist (no target). | |||||||||||
How this is counted. There is no e-consent project
for SPINAPS, so a participant is counted as recruited in the month of
their eligibility assessment (elg_doa, Eligibility
Checklist), the first study procedure after consent. Participants
without an eligibility date are left out of these plots (5 at
present).
How this is counted. The protocol recruits mainly from SPIN-ASD and SPIN-R, and also from TAY, SYNAPSIS, a cannabis and alcohol PET study, CAMH clinics and referrals, and advertising. REDCap records only the first three: a participant is counted under SPIN-ASD or SPIN-R when they are linked to a record there (the IDs on the Shared Data Availability form, checked against the origin project, or the matching SPN40 number; see Data Quality, Origin IDs). Participants in both are shown once, under SPIN-ASD and SPIN-R. TAY is not pulled (no API access), so TAY participants are counted only when the Shared Data form ticks TAY. Not linked covers everyone else: other studies, clinics, advertising, or an origin ID that is missing or wrong. The source of those participants is not recorded in REDCap.

How this is counted. A participant is withdrawn when
the Early Termination/Withdrawal Checklist says they were prematurely
withdrawn (term_premature_yn); the reason is the one ticked
on that form. This includes participants who never received a group, so
they appear under Not assigned.

Summary of who is in the study, where they came from and whether they completed it, for the SPINAPS paper.
What REDCap can and cannot show. The flow starts at a SPINAPS record, which is created at consent. Who was contacted, who was interested and why people did not consent are recorded only in the screening log, which is not in REDCap, so those boxes of a CONSORT diagram cannot be filled from here.
How each step is counted.
config/eligibility_criteria.csv), or that
the checklist is not filled in.Each column follows the participants who came from one study. When a participant was in both SPIN-ASD and SPIN-R, they are counted under SPIN-R (the later study, and the preferred source on the Shared Data form). Beside each arrow: who dropped out at that point and why, or who is still waiting.
Please note : PET data missing refers to pending QC. The final completed number is calculated based on having all data points completed - from cog scales, PET QC, etc.
One row per participant: where they came from, their status, and the data still in progress for anyone scanned.

What is shown. Everyone with a SPINAPS record, by group, using the SPINAPS Demographics and Height/Weight/Vitals forms. Gender is shown as recorded (check all that apply; none of the current participants ticked more than one). Mother’s education has two fields on the form and either is used. Handedness and parental education are only filled in for a few participants in SPINAPS; the From origin study tab adds them from the study each participant came from. Values entered as REDCap missing-data codes (e.g. ASKU) or outside their valid range count as missing. Current SSRI and antipsychotic use are added because the protocol matches the groups on them (3.2.1, with age, sex, gender and handedness); they come from the psychotropic medication form and are missing when that form is not completed (see Medications).

What is shown. The spread of the main descriptive variables by group, for everyone with a SPINAPS record (one bin = one year of age or education; 5 IQ points). Groups are overlaid, so where they overlap the colours mix. IQ comes from the origin studies (see Data Sharing), because no IQ has been entered in SPINAPS REDCap yet.
Most SPINAPS participants took part in SPIN-ASD or SPIN-R first, and the protocol reuses their assessments and MRI instead of repeating them. This section shows where each participant’s data came from, using the Shared Data Availability form and the origin studies’ REDCap projects.
How this is counted. The studies ticked on the Shared Data Availability form, separately for clinical data and MRI. A participant who was in SPIN-ASD and then SPIN-R can have both ticked.
| Clinical data shared from | ASD-Impaired | ASD-Intact | TDC | Not assigned | Total |
|---|---|---|---|---|---|
| None recorded | 0 | 0 | 2 | 4 | 6 |
| SPIN-ASD | 7 | 0 | 5 | 0 | 12 |
| SPIN-ASD + SPIN-R | 5 | 1 | 1 | 0 | 7 |

Why it matters. SPINAPS does not repeat the MRI when a good-quality one exists from the referring study. Multi-shell diffusion is needed for the NODDI analyses in the exploratory aim, so it is counted separately.

How this is read. One row per participant, one column per form given at the SPINAPS assessment event, in REDCap’s order, read from the REDCap project itself (no hand-kept form list). Each cell is the form’s “Was this form completed?” answer. Green = completed, gold = partly completed, red = recorded as not completed, grey = nothing entered. Forms without that question (AQ, ABAS-III) use REDCap’s own form status: “Incomplete” is gold when the form has answers and grey when it was never opened. Dates, rater, REDCap ID, notes, mode of administration, validity questions and calculated fields don’t count as answers, and an unticked checkbox doesn’t either. The two ABAS-III forms (summary scores and questionnaire) share one column. Logs (withdrawal checklist, progress notes, adverse events, change log) are left out. Hover over a cell for the recorded answer and, for forms marked not completed, the reason. Not shown: SCID-5. The REDCap API returns no fields from this form, which usually means the API user has no export rights on it.
How this is read. Whether a score for each
instrument exists in the participant’s linked SPIN-ASD or SPIN-R records
(see Data Sharing), and how old it is at the SPINAPS PET.
Green = reusable under the protocol (3.4): within 6
months of the PET, or an instrument reused whatever its age (IQ, ADOS-2,
AQ, handedness, parental education); gold = older than
6 months, or no assessment date; grey = no origin
score. Each row is labelled with the SPN40 ID and the origin record(s)
it is linked to (SPIN-ASD SPASD_..., SPIN-R
SPN3x_...). Hover over a cell for the study, origin ID,
visit and assessment date the score came from, or, for a grey cell,
which linked records were searched. This is what could be uploaded to
SPINAPS now, and what would need re-assessing.
What is checked. Whether each ASD participant’s
recorded subgroup (Eligibility Checklist, elg_group)
matches the protocol rule (3.3.2, version 5). ASD-Impaired means at
least one of: MATRICS composite or RMET total at least 1 SD below the
mean of the SPIN-ASD and SPINS typically developing controls aged 16-35,
or IQ < 90. Otherwise ASD-Intact. The rule is applied to every
participant, including those grouped before the protocol changed.
Why only cognition. Earlier, the rule also used the
Birchwood SFS and ABAS-III; the protocol dropped them because too many
participants were impaired on SFS. The REDCap Eligibility Checklist
(elg_impaired, elg_comp_assess) still lists
SFS and ABAS-III.
The cut-offs are the values the PI computed from
those controls at the start of the study
(config/group_cutoffs.csv). They are not recomputed here,
because the control data have changed since.

Bars show the number of participants in each group with each SCID-5 diagnosis. A participant with several diagnoses is counted once for each.
No current / lifetime diagnosis collects everyone with a SCID but no diagnosis meeting that tab’s definition; the hover gives the reason (the first that applies): no clinical disorder on the SCID; past disorder, no longer current; lifetime disorder, current rating not entered; below threshold only (subthreshold, absent or inadequate information); disorder selected, ratings not entered; disorder said to be present, but none selected. Gaps in data entry are also listed under Data Quality.
Known exceptions. In the SPINAPS / SPIN-R layout, disorders without a lifetime rating (e.g. premenstrual dysphoric, sleep-wake and somatic disorders, adult ADHD) use their current rating for both, so they never appear as Past only. In SPIN-ASD, Unspecified feeding or eating disorder has no current rating, so it is always Past only.

From origin study: the SCID closest in time to the
participant’s SPINAPS PET among their linked SPIN-ASD and SPIN-R visits,
decoded with that study’s score sheet. The protocol reuses the SCID
history when it is older than 6 months but re-assesses current status,
so Current from an old SCID may be out of date. Decoding is the
same as in the SPIN-R dashboard
(config/scid_codes_spinr.csv,
config/scid_codes_spasd.csv).
The SCID-5 form is not exported by the REDCap API (no fields from it are returned, which usually means the API user has no export rights on it).

Each instrument has two tabs: SPINAPS REDCap (scores entered in SPINAPS) and From origin study (the participant’s linked SPIN-ASD or SPIN-R assessment closest to their PET; see Data Sharing). Every participant is a point; hover for the participant and, in the origin tab, which record and visit the score came from and how old it was at the PET.
Self-report of autistic traits (50 items; total 0-50, higher = more traits).
No scores entered in SPINAPS REDCap yet.
Observational assessment of autism; given to ASD participants only. Calibrated severity scores (CSS, 1-10) are comparable across modules: overall, social affect (SA) and restricted and repetitive behaviours (RRB).
No scores entered in SPINAPS REDCap yet.
Clinician-rated psychopathology (18 items, each 1-7). Shown as the total and the five factor scores used in the SPIN studies.
No scores entered in SPINAPS REDCap yet.
Self-reported depression (PHQ-9, 0-27) and anxiety (GAD-7, 0-21) over the past two weeks. 5, 10 and 15 mark mild, moderate and (moderately) severe on both. These are SPINAPS-only questionnaires, so there is no origin tab content.
Not collected in the origin studies (SPINAPS only).
Full-scale IQ (standard score, mean 100, SD 15): the WAIS-IV when it was given, otherwise the WASI-II 4-subtest FSIQ, otherwise the 2-subtest FSIQ. IQ < 90 is one of the ASD-Impaired criteria (see Group Assignment Check). The protocol reuses IQ from the referring study regardless of age.
No scores entered in SPINAPS REDCap yet.
T-scores (mean 50, SD 10) for the seven cognitive domains and the overall composite. The composite is one of the ASD-Impaired criteria (see Group Assignment Check).
No scores entered in SPINAPS REDCap yet.
Choosing which of four words best describes the mental state shown in 36 photographs of the eye region (total 0-36, higher = better). A total at or below the cut-off is one of the ASD-Impaired criteria (see Group Assignment Check).
No scores entered in SPINAPS REDCap yet.
40 faces, each happy, sad, angry, fearful or neutral: correct responses in total and per emotion (8 faces each).
No scores entered in SPINAPS REDCap yet.
Adaptive behaviour composites (standard scores, mean 100, SD 15): general adaptive composite (GAC) and the conceptual, social and practical domains. ABAS-III is no longer an ASD-Impaired criterion (protocol version 5).
No scores entered in SPINAPS REDCap yet.
What is shown. Participants whose PET Scan Information form is completed, by group: injected dose and mass, the arm used for the arterial line, samples collected, and whether the participant had side effects or complications from the arterial line or the scan. Technician notes are from the PET QC form (notes recorded for the scan or the blood draw). These are SPINAPS procedures, so there is no origin-study tab.
Specific activity (at synthesis and at injection) is not shown: the REDCap fields have no units and the values recorded differ by orders of magnitude, which suggests more than one unit is in use (see Data Quality). Dose and mass values outside their valid range (see Data Quality, Values) are left out and listed there.

Outside the range, not a number, or a missing-data code. Scores from both sources, and the other checked SPINAPS fields, against their valid range (table below). These values are left out of every table and plot. REDCap missing-data codes (e.g. ASKU) are read as missing and listed for completeness. Source is SPINAPS REDCap, the origin scores used in this report, or the origin record where a value that is not a number was found.

Scores of 0 treated as not administered. A TASIT-R part total of 0, or an ER-40 total, happy, angry or neutral score of 0, cannot come from a test that was taken (the same rule as the SPINR dashboard). These scores (for an ER-40 total of 0, all ER-40 scores) are left out; confirm the test was not given, or correct the score.
No scores of 0 treated as not administered.
Ranges used. From each field’s REDCap validation
where it has one; otherwise from config/score_ranges.csv,
which says how the range was worked out. Origin-study scores are checked
under their SPINAPS names (the validation is the same in SPIN-ASD and
SPIN-R).

Specific activity. The PET form’s specific-activity fields have no units, and the values span several orders of magnitude. Values more than 10 times away from the field’s median are listed: probably a different unit (e.g. GBq/µmol vs mCi/µmol). These fields are not used in this report until the unit is agreed.

Totals from forms not marked completed. REDCap’s total-score calculations add up whichever items were answered, so a total from a form that is partly filled in, or marked not completed, can be too low. Listed: calculated totals that are range-checked here, whose form is not marked completed. They are still used; complete or correct the form.
No totals from forms that are not marked completed.
Checks on each form’s “Was this form completed?” answer (or REDCap’s own form status, for AQ and ABAS-III) against what is entered, from the same data as the Assessment Completion heatmaps. Dates, rater, notes and other administrative fields don’t count as entered data. A form version marked not completed is not checked when the other version of the same form (ABAS-III) is marked completed.
Marked completed, but no data entered. The form says it was completed, but none of its questions are answered. Enter the data, or correct the answer.
None.
Data entered, but not marked completed. Questions are answered, but “Was this form completed?” is empty (or REDCap’s status is Incomplete). Answer it.
None.
Marked not completed, but has data. The form says it was not completed, but has answers. If it was partly done, mark it Partial (where the form allows it).
None.
Marked not completed, no reason given. The form says it was not completed, but “Why was this form not completed?” is empty.
None.
Reason “Other” with no explanation. The reason is “Other”, but the explanation is empty.
None.
No group or eligibility. Participants with no group on the Eligibility Checklist, or not recorded as eligible. They are counted as Not assigned throughout.

Enrolled outside age 16-35. The protocol includes participants aged 16-35. Age at the eligibility assessment, from the date of birth (or the Demographics age at study entry when a date is missing).
Every enrolled participant was 16-35 at eligibility.
Enrolled with IQ < 70. IQ < 70 (WASI-II or WAIS-IV) is an exclusion criterion for everyone. Listed: enrolled participants with a full-scale IQ below 70 in SPINAPS or the origin study.
No enrolled participant has IQ < 70.
TDC to confirm. Typically developing controls must have no DSM-5 disorder, except adjustment disorder, phobic disorder, or a single past major depressive episode at least two years ago and not currently treated. Listed: TDC with any lifetime diagnosis on their origin SCID, or a current psychotropic medication in SPINAPS. Whether an exception applies is for the study team to confirm.
| Participant | Issue | Detail |

SCID data entry gaps. From the SCID section’s per-disorder tables, for each source: a disorder selected with its ratings left empty; a disorder at lifetime threshold whose current rating (asked because of that) is empty; “clinical disorder present” answered Yes with no disorder selected. For the origin study, the SCID used is the one closest to the PET.
| Check | Participant | Group | Source | Disorder |
|---|---|---|---|---|
| Lifetime threshold, current rating not entered | SPN40_CMH_0116 | ASD-Impaired | From origin study | Posttraumatic stress disorder |
