Executive Summary

This report presents a consolidated national summary of Uganda’s community health status and service delivery by Village Health Teams (VHTs) based on data reported through the HMIS 097b Quarterly Report for Quarter 4 of 2025. The findings represent a critical snapshot of primary health indicators managed at the community level and provide key insights into child health, maternal services, immunisation, disease surveillance, and essential household practices across the country.

National Summary of Key Indicators

A total of 87,208 were expected to report through through DHIS2 for Q4 2025. A total of 5,608,113 households were registered to date. The data coming from Bugweri, Buvuma, Fort Portal City, Hoima City, Kabarole, Kamwenge, Kapchorwa, Kyotera, Luuka, Ngora, Rwampara, Sembabule, and Soroti City will be integrated in the next issue.

Reporting Rates

This is Calculated as the Actual Reports received as the numerator and Expected Reports as the Denominator.

A total of 149,152 people with functional Limitations were reached by CHWs for the quarter under study.

Mortality

A total of 66,210 were reported as dead in the quarter of April to June 2025. Majority of these were children 48,271 (72.9%) in the age category of 1 year to 5 years.

Reproductive Health

Registration of Pregnant Mothers

Community Health Workers (CHWs) reported 54,464 pregnant women with at least four ANC visits and 116,619 with eight visits between April and June 2025.

Immunisation

A total of 1,564,293 children under 5 years were assessed by CHWs for immunisation status and were found to have up to date immunisation.

CHWs reported adolescents and young people 1,008,894 and 458,405 to have received TD vaccine and HPV vaccine between April and June 2025.

Recommendations

  1. Prioritize community health supply chains—ensure zero stockout of RDTs, ACTs, and essential antibiotics.

  2. Invest in refresher training for VHTs on emergency care, nutrition, and maternal health.

  3. Expand community-based immunisation and nutrition campaigns.

  4. Provide financial and digital tools to support real-time VHT data reporting.

  5. Strengthen supervision, linkage to facilities, and follow-up for referred patients.

This evidence-based report calls for strategic investment and renewed commitment to community health—especially in light of Uganda’s ambition to achieve universal health coverage and reduce under-5 mortality.
