2.1 General Application (DS-11)

3 Child Passport Application (Under 16)

3.1 Parental Applications

For cases where both parents/guardians cannot apply, the following forms need to be notarized:

1. One parent available form DS-3053 PDF
2. Sole custody or single parent form DS-3053 PDF
3. Unlocatable other parent form DS-5525 PDF

Important: Do not submit the application online without ensuring all information is correct and all relevant documents are saved and organized. If there is a typo you will have to start over.

3.1.1 DO NOT SUBMIT APPLICATION:

• 1. Download the forms you need, save them to a folder on your computer or drive, add to folder all relevant and required paperwork to keep together (birth certificate, social security, id’s, divorce decrees, etc)
• 2. Print completed application – save as PDF to your computer.
• 3. Email a copy to yourself and spouse.
• AGAIN - DO NOT SUBMIT FOR PROCESSING.
