Thoracic Surgical Training and Course Design
Dear Thoracic Trainee,

We are designing a new course for Thoracic trainees. We would like this course to be based on any skills that would be sought by thoracic surgery trainees, which are currently missing in current training programmes.

In order to do this, we would like to assess which skills would be most beneficial for trainees to learn in courses, to augment their current teaching.

The following technical skills and procedures have all been taken from the ISCP Thoracic Surgery Curriculum.

All feedback is this form is anonymised.

Mr Sasha Stamenkovic
Robotic Lead and Thoracic Surgery Consultant, St Bartholomew's Hospital
Consultant Founder and Thoracic Lead, LSOC

Difficult for core surgical trainees to scrub in for robotic cases without robotic training
Easy cases which used to be for training are going to be done by robotic now
Lists are shifting from VATs towards the robotic so I got less chance of training as I am only changing the arms
We lost two days for robotic list. We don't get benefit from robotic.
In previous trust the robotic programme was new with no second console so we were used as bedside assistants. At current trust, the robotic training is good as there is lots of opportunity, and there is a second console and dedicated robotic assistants.
Enforced the operative technique for performing different anatomical lung resection regardless of the approach.
Easier cases e.g. wedges would be done on the robot, which would ordinarily be training cases.
Trainees not involved in robotic cases other than as bedside assistant
Current unit very few trainers on robot due to consultants training. I lost a list a week. Now I'm starting to be trained and my next unit I will be trained doing all cases robotically. I feel the temporary hindrance has been necessary and will be net positive in the future
N/A
Less cases offered VATs
Training partly improved as it has offered a new skillset to learn from. But it has reduced the number of VATS cases available to learn on.
Fewer training lists for trainees
Most cases are robotic
Robotic theatre sessions I only allowed to assist only in sitting up in the present of SCP (we have one robotic theatre a week out of four)
At my stage need to learn open/VATs
Clicked 3 - no effect as no robot at current unit. Previous unit would have to be at table doing robot arms if robotic case. Only had one list a week and part of list could be robotic.
Na
When robotic assisting, very little to do. Unless there is an extra assistant and extra robot, no chance for robotic training
At my current job, as it is only consultants operate on the robot, and some even training themselves, there is no scope for trainees to learn. As there is no scope to learn, you are at assisting role only. Plus, if you are very junior, the consultants choose SCP over you, instead of teaching you how to set up and assist.

Difficult to get cases
The same as above
Lists shifted towards the robotic at least 6 list a month
We lost 2 VATS/open list weekly.
Again, depends on trust you are at. At the previous trust the consultants were learning the robot so we lost vats lists due to this. At my current trust, my consultant does both VATS and RATS so there are excellent opportunities for both.
The visualisation you achieve from Robotic approach helps you build a deeper understanding when utilising VATS approach or even open..
Less VATS/Open cases - that could be trainee led are performed. Now often for emergencies or complex cases which elements of a case are done
Dedicated robotic lists no longer have training cases
I went on an OOPT to do VATs so I will be trained fully in both.
N/A
Simpler cases are now robotic
More cases switched to RATS instead of VATS.
Fewer training lists in VATS a consultants learning robot
Very little VATS given robotic lists
If robotic session was a VATS I might been offered to do cases in this
Lists taken up by robotics means less to do for VATS/open
Clicked 3 - no effect as no robot at current unit. Previous unit would have to be at table doing robot arms if robotic case. Only had one list a week and part of list could be robotic.
Na
Always opportunity for trainees
If the consultant has a robot list instead of anything else, and you are paired to be on that list - no training would happen. Hence no VATS or open. People are very enthusiastic to put you on a robotic list, without realising how little actual training happens there. If you’re very junior - it is the same as watching a video online, since noone allows or teaches you how to set up, and you do not even dream on actually doing anything on the console.

Able to do uniportal surgeries and consultants train well with Vats hands off technique
Difficult cases is done open, easier cases are done by VATs.
Open surgery now seldom, if so, it would be so complicated that done by the consultant.
VATS helped me to understand better the anatomy and improved my surgical skills.
Just get much less open cases, and they are few and far between.
No more Pneumonectomies or open lobectomies.. how I am going to learn how to control the main PA
Open left for complex cases, and so few and far between.
Open cases already rare and barely affected
I've worked with purely open consultants before VATS. There are also extended resections and sleeves which have been done open to allow open training.
Open surgeries performed rarely and only for very complex cases or complications during VATS, in which cases the consultant usually needs to take over. Trainees in our department feel more confident performing VATS procedures than open (as discussed many times).
Open cases are less frequent but still occur
Very few open cases now performed. Rare opportunities to learn open cases or perform thoracotomies. This is off concern considering that open surgery should be the emergency recovery procedure, but increasingly more trainees cannot do it.
Slightly different skill set but able to mix abd match during open procedures.
Na
Open has became restricted to complex cases where consultants are more keen to start this cases from the beginning
Lists taken up by robotics means less to do for VATS/open
2 units in our deanery - 1 does majority open lobes and 1 VATS (now with robot) so feel good mix of open and VATS
When there are open cases boss does them and they’re usually. Complex so not a good training case
Do not open frequently
There is more VATS than open, but since the two approaches needs to be learned in parallel, I do not see this as an issue.

Principles of setting up ( positioning, post placement, incision placement) and closure (specimen retrieval techniques, closure of thoracotomy, drain placement, etc. ), since these are the procedures that you start from when you begin as a surgeon at the early stages. You usually learn them by doing, but for some things knowing the idea behind why we do what we do - would help to make more independent decisions and lead to independent work faster. Uniportal techniques - used not only for resecton, but also for minor procedures. More often for minor procedures, where you start as a junior. Knowing how to navigate instruments in uniportal would really help to progress. Response to emergency situations - how to convert from RATS/VATS to open in case of emergency, what to do in case of bleeding, techniques to apply pressure, what to do in case of cardiac arrest when patient is in lateral decubitus. Airway emergencies - what can you do in immediate response on the ward, or in theatre. In case of loss of ventilation.

High fidelity simulation that is easily accessible would be of huge benefit. The robotic simulation exercises I have seen are basic but valuable, to get advanced versions specifically for thoracic surgery would be phenomenal for training.

Would be interesting if the courses could advise on techniques or ways to practice what you learn. Since if you do not get the opportunities at work to learn, going on a course is unlikely to make it better. And you loose the skills if you do not practice. So most frustration comes from learning something new, wanting to apply it in practice, not getting that chance, and forgetting it. Hence if there are ways, that you can do simple models, practice at home/in sim lab, those advises would be very useful.
