Table 1: Basic demographic and cognitive information for AD and DLB groups.

Figure 1

Figure 1: (a) Baseline and 6 month daily activity counts for AD and DLB participants averaged over 14 days of wear-time for each time period. Lines connect the baseline and 6 month values for each subject. (b) Boxplots displaying the difference in daily average activity counts (6 month - baseline) for AD and DLB groups. All units are x10^6.

Figure 2

Figure 2: (a) Baseline and 6 month night/day activity ratio values for AD and DLB participants averaged over 14 night/day pairings of wear-time for each time period. Ratios were calculated by dividing the average activity level of each night (11:00pm-5:00am) by the average activity level of the following day (11:00am-5:00pm). Lines connect the baseline and 6 month values for each subject. ** denotes a subject with a large change in night/day activity ratio value from baseline to 6 month. (b) Boxplots displaying the difference in night/day activity ratio values (6 month - baseline) for AD and DLB groups.

Figure 3

Figure 3: Diurnal activity plot that displays the activity counts per minute for every minute of the day averaged over the 14 day wear period for a single subject at both baseline and 6 months. Loess curves summarize the general pattern of activity throughout the average day. Red shading represents the nighttime period (11:00pm-5:00am) and blue shading represents the daytime period (11:00am-5:00pm).

Figure 4

Figure 4: Boxplots depicting the baseline and 6 month values for the percent of sleep time spent in (a) REM (b) non-REM hypertonia (c) deep N2 and (d) atypical N3 for both AD and DLB groups. Values were calculated by averaging each measure over two nights of sleep for which Sleep Profiler devices were worn by participants.

Results

Patient Characteristics (Table 1) Probable AD (n = 6) on average 10 years older than probable DLB (n = 8) participants Both groups mostly male, reflecting recruitment from VA Both groups have similar baseline MoCA scores

Actigraphy (Figures 1 – 3) Both groups tended to have similar activity count (AC) levels at baseline (Figure 1a); DLB group tended to have declines in activity between baseline and 6 months, whereas AD group tended to maintain levels (Figure 1b) AD: median daily AC difference = 0.0x10^5 (IQR = 1.2x10^5)
DLB: median daily AC difference = -3.2x10^5 (IQR = 3.8x10^5) Both groups tended to have similar night/day activity ratios at baseline (Figure 2a), with similar declines in night/day ratio between baseline and 6 months (Figure 2b) AD: median night/day ratio difference = 0.0 (IQR = 0.1) DLB: median night/day ratio difference = 0.0 (IQR = 0.1) One DLB participant exhibited a large change in night/day activity ratio from baseline to 6 month (marked ** in Figure 2); their diurnal activity plot (Figure 3) shows large change from higher activity during the day at baseline to higher activity at night at 6 months

Sleep (Figure 4) Traditional Measures: REM, non-REM hypertonia (NRH) AD group tended to have greater time spent in REM sleep than DLB group at baseline (AD median = 20%; DLB median = 3%) with neither group experiencing substantial changes from baseline to 6 months (Figure 4a) AD group tended to spend less time in NRH than DLB group at baseline (AD median = 2%; DLB median = 12%) with both groups tending to spend more time in NRH at 6 months (Figure 4b) Novel Measures: deep N2, atypical N3 (AN3) Both groups tended to have similar baseline time spent in deep N2, and AD group tended to increase from baseline to 6 months whereas DLB group tended to decrease (Figure 4c) AD: median %deep N2 difference = 2% (IQR = 2%) DLB: median %deep N2 difference = -4% (IQR = 8%) DLB group tended to have larger baseline time spent in AN3 and tended to increase from baseline to 6 months while AD group tended to maintain levels from baseline to 6 months (Figure 4d) AD: median %AN3 difference = 0% (IQR = 2%) DLB: median %AN3 difference = 2% (IQR = 3%)
