Medical Breach Summary

This dataset contains information on breaches of unsecured protected health information (PHI) reported to the US Department of Health and Human Services (HHS) in the Office for Civil Rights (OCR) since 2009. The OCR is responsible for collecting and reporting disclosures of PHI as required by law. The data reported for each breach include the name of the covered entity (organization responsible for the PHI), state where the breach was reported, covered entity type, number of individuals affected, breach submission date, type of breach, location of breached information, whether a business associate was present, and a web description explaining what happened and the resolution. The goal of this analysis is to identify trends in how companies are being breached and where these breaches occur most frequently.
